All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. FOUND ABOUT 30 MOUSE DROPPINGS IN DRY FOOD STORAGE AREA AND UNDER FRONT SERVICE LINE. ONE POSSIBLE ENTRY HOLE NOTED UNDER FRONT COUNTER, INSTRUCTED MGR TO ELIMINATE RODENT'S ACTIVITY AND SEAL ALL HOLES.
The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. LEAKY DRAIN PIPE UNDER REAR PREP AREA HAND SINK, INSTRUCTED TO REPAIR
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. MUST PROVIDE A WORKING THERMOMETER IN ALL COOLERS WHERE PERISHABLES ARE STORED.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MUST STORE SODA CASES ELEVATED 6' OFF FLOOR.
A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. NO CERTIFIED FOOD AND SANITATION MANAGER ON SITE WHILE COOKING AND SERVING POTENTIALLY HAZARDOUS FOODS, INSTRUCTED TO OBTAIN.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. The walls and ceilings shall be in good repair and easily cleaned. CRACKS ON KITCHEN WALL COVERINGS, INSTRUCTED TO SEAL.

FOUND FRONT PREP COOLER NOT ABLE TO MAINTAIN THE PROPER TEMPERATURE OF 40F OR LOWER, AIR TEMP. OF 56.1F. INSTRUCTED TO HAVE COOLER SERVICED SO AS TO MAINTAIN THE PROPER TEMPERATURE. UNIT WAS TAGGED AND CANNOT BE USED UNTIL FIXED. CITATION #70239-14
FOUND POTENTIALLY HAZARDOUS FOODS BEING STORED IN THE ABOVE COOLER AT IMPROPER TEMPERATURES. TEMPERATURES RANGED FROM 50.9F TO 113.3F AND INCLUDED ITEMS SUCH AS CRAB CAKES, RICE, CHEESE, MILK, CHICKEN, BEEF, JAMBALAYA, GUMBO, COOKED PASTA, FISH, CABBAGE, SHRIMP AND BEANS. ALL ITEMS WERE DENATURED AND DISCARDED. A COMBINE WEIGHT OF APPROXIMATELY 20LBS VALUED AT $50.00. NSTRUCTED TO KEEP COLD FOODS 40F OR LOWER.
MUST PROPERLY DATE AND LABEL COOKED/RAW POTENTIALLY HAZARDOUS FOODS THAT HAVE BEEN REMOVED FROM THEIR ORIGINAL PACKAGING IN WALK IN FREEZER AND COOLER.
MUST REMOVE FOIL FROM COOKING EQUIPMENT.
