All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Door gasket of 3-door cooler facing the oven(middle door) that's in poor repair, must be fixed.
The walls and ceilings shall be in good repair and easily cleaned. Missing baseboard behind 3-comp sink must be replaced.

All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. PREP COOLERS DOOR RUBBER GASKETS RIPPED. MUST REPAIR OR REPLACE DAMAGED DOOR GASKETS.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED FOOD BUILD UP ON FLOOR UNDER AND AROUND COOLERS AT PREP AREA. MUST DETAIL CLEAN FLOOR THROUGHOUT AND MAINTAIN.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. 3-COMPARTMENT SINK FAUCET LEAKING. MUST REPAIR AND MAINTAIN SINK.

All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. INSTRUCTED MANAGER TO PROVIDE AND CALIBRATE METAL STEM THERMOMETER FOR FOOD HANDLERS.
