The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CLEAN FLOOR OF ELECTRICAL ROOM.
The walls and ceilings shall be in good repair and easily cleaned. REPLACE WALL TILES IN PREP AREA AROUND STOVES AND BURNERS. REPAIR THE WALL BEHIND AND UNDER THE DISHMACHINE CLOSE OPENINGS, MAKE SMOOTH AND CLEANABLE.CEILING OF WALK IN COOLER MUST BE SMOOTH AND CLEANABLE.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST INSTALL BACK FLOW PRVENTION DEVICE ON MOP SINK. MUST REPAIR URINAL SO ALL WASTE LIQUID COMPLETELY FLUSHES DOWN DRAIN.

A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. PREVIOUS MINOR VIOLATIONS FROM 10-16-09 #163323 AND SHORT COMPLAINT INSPECTION ON 1-8-10 #154227 NOT CORRECTED. #33-MUST CLEAN INTERIOR OF ALL REACH IN COOLERS.#35-DAMAGED WALL TILES AT BOTH STOVES, WALL UNDER DISH MACHINE WITH OPENINGS. THE CEILING IN THE WALK IN COOLER NOT SMOOTH, CLEANABLE WITH OPEN CREVICES. MUST REPAIR ALL.#38-NO BACK FLOW PREVENTION DEVICE ON UTILITY MOP SINK. MUST PROVIDE OR REMOVE HOSE AND THREADED NOZZLE.#38- MUST REPAIR URINAL SO THAT ALL WASTE WATER COMPLETELY FLUSHES DOWN DRAIN.
All cold food shall be stored at a temperature of 40F or less. FOUND APPROXIMATELY 10LBS COOKED LAMB CURRY IN THE ABOVE MENTIONED TWO DOOR COOLER AT 48.9F AND 5LBS COOKED CHICKEN CURRY AT 46.9F. ALL FOODS DISCARDED.

TWO DOOR COOLER NEAR DISH MACHINE FOUND AT 44.9F AND FOODS SUCH AS LAMB CURRY AT 48.9F. DOORS OF COOLER IN POOR REPAIR WITH BROKEN GASKET SEALS AND CROOKED DOORS UNABLE TO CLOSE PROPERLY. COOLER TAGGED HELD FOR INSPECTION AND MUST NOT USE UNTIL REPAIRED, DOORS CLOSE PROPERLY AND MAINTAINS TEMPERATURE OF 40F. MUST CONTACT C.D.P.H. SUPERVISOR SALAZAR @312-746-5426 FOR TAG REMOVAL. ALSO UNUSED COOLER ON COOKS LINE TAGGED HELD FOR INSPECTION. TEMPERATURE AT 55F. MUST NOT USE UNTIL REPAIRED.

NO CHLORINE SANITIZING RINSE WAS DETECTED AT THE LOW TEMPERATURE DISHMACHINE. UNIT WAS NOT IN USE DURING THE INSPECTION. MUST MAINTAIN A FINAL RINSE OF 100 PPM CHLORINE. ECOLAB TECHNICIAN ARRIVED ONSITE AND GAVE THEM A NEW BUCKET OF SANITIZER FOR THE MACHINE. FINAL RINSE OF 100PPM AT END OF INSPECTION. NO CITATION ISSUED. CRITICAL VIOLATION 7-38-030.
NO PAPER TOWELS WERE PROVIDED AT THE EXPOSED HANDSINK IN THE PREP AREA AND AT THE EMPLOYEE WASHROOM. MANAGEMENT PROVIDED PAPER TOWELS AT MY REQUEST. CRITICAL VIOLATION 7-38-030.
FOOD (ICE) NOT PROTECTED FROM CONTAMINATION DURING STORAGE INSIDE OF THE ICE MACHINE. OBSERVED MOLD GROWTH ON INTERIOR CONTACT SURFACES OF THE ICE MACHINE. MANAGEMENT INSTRUCTED TO TURN OFF MACHINE, REMOVE ICE AND HAVE INTERIOR SURFACES AND WATER HOSES CLEANED AND SANITIZED. SERIOUS VIOLATION 7-38-005A.
