All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines.OBSERVED EVIDENCE OF MOUSE DROPPINGS APPX. 100 OR MORE BEHIND COOKING EQUIPMENT,IN LOWER CABINETS,PREP TABLES,ALONG WALL BASES.REAR GARBAGE DOOR NOT TIGHT FITTING ALONG BOTTOM.ALSO NOTED SEVERAL OPENINGS IN WALLS THROUGHOUT.MUST REMOVE DROPPINGS ,CLEAN AND SANITIZE AREAS,RODENT PROOF DOOR.,SEAL HOLES.RECOMMENDED TO HAVE A PEST CONTROL OPERATOR TO SERVICE PREMISES ON OR AFTER 2/1/10.

DEFROST AND CLEAN THE UPRIGHT FREEZER HAS ICE BUILDUP LOCATED IN THE PREP AREA. CLEAN THE HOOD AND FILTERS ABOVE THE COOKING EQUIPMENT HAS GREASE BUILDUP ON THEM.
CLEAN THE FLOOR IN THE BASEMENT HAS DUST BUILDUP ALONG YHE WALLS AND IN THE CORNERS.
NO CERTIFIED FOOD MANAGER WITH THE CITY OF CHICAGO SANITATION CERTIFICATE ON SITE WHEN POTENTIALLY HAZARDOUS FOODS ARE PREP AND SERVED. STEAK,SHRIMP.SERIOUS CITATION GIVEN 7-38-012
