All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.OBSERVED IN ROOM #6, STAINLESS BOWL,WATER PITCHER,AND GLASSES STORED ON DRAINBOARD.ITEMS ARE WASHED IN CLASSROOM(PER TEACHER COMMENT).INSTRUCTED CERTIFIED MANAGER THAT THOSE ITEMS MUST BE WASHED, RINSED,AND SANITIZED AT 3 COMPARTMENT SINK IN KITCHEN.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. NEED TO PROVIDE THERMOMETERS FOR REFRIGERATORE IN CLASS ROOMS(#6-#7-#8).DRINKS FOR CHILDREN ARE STORED IN IT.
All employees must wear garments that are clean and of washable character and nature. All employees shall be required to use effective hair restraints to confine hair. OBSERVED STAFF MEMBERS IN AND OUT THE KITCKEN WITHOUT HAIR RESTRAINANT.ANYONE ENTERING PREP AREA(KITCHEN)MUST WEAR A HAIR RESTRAINANT.

All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. CLEAN ENCRUSTED FOOD AND ICE BUILD-UP IN FREEZER OF STAFF DINING AREA.(REFRIGERATOR TOP PART)
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.OBSERVED THREAD FAUCET AT 3 COMPARTMENT SINK, IN PREP AREA.INSTRUCTED TO PROVIDE UN THREAD FAUCET.

NEED TO REPAIR WATER HANDLE AT BOYS WASHROOM,2ND FLOOR( NORTH). NEED TO UNCLOG SLOW DRAINING WASHBOWL IN WOMEN'S WASHROOM(NEXT TO PIPE;SAME WASHROOM THE OTHER WASHBOWL(WALL MUST BE REPAIRED BEHIND THE WASHBOWL).

MUST CLEAN AND SANITIZE STICKY FLOOR IN BOYS BATHROOM 1ST FLOOR NORTHSIDE OF BLDG.
MUST DEFROST FREEZER AT KITCHEN TO REMOVE EXCESS ICE AND FROST BUILD-UP. MUST ALSO CLEAN INTERIOR OF BOTH MICROWAVE OVENS IN THE TEACHER'S LOUNGE TO REMOVE FOOD STAINS.
MUST REPAIR OR REPLACE TOILET WATER TANK LID AT WMONE'S STAFF TOILET ROOM ON THE 1ST FLOOR WESTSIDE OF BLDG.
MUST REMOVE PEELING PAINT AND REPAINT WALL NEXT TO HANDWASHING SINK AT WOMEN'S STAFF RESTROOM ON 2ND FLOOR SOUTHSIDE.
