Potentially hazardous foods does not meet temperature requirement during storage, preparation, display and service. Found prepared egg salad in bulk container and individual packaged containers on prep table at improper temperature. Temperature of egg salad 25 lbs at 46.5F to 45.5F. Instructed that potentially hazardous foods must maintain 40F or below at all times. Manager voluntarily discarded and denatured. Value $150.00. Critical citation issued 7-38-005(A).
Light bulbs not shielded inside reach in freezer and in prep kitchen of assisted living. Instructed to shield bulbs or have shatter proof.
Floors in gift shop, corners and behind sinks, not cleaned. Instructed to detail clean.
Wall above 3 compartment sink has peeling paint. Also, peeling paint on walls in dishmachine room. Instructed to remove peeling paint and repaint wall. Ventilation and fan covers thru-out ceiling and walk-in unit not cleaned. Instructed to detail clean. Ceiling tile in assisted living prep kitchen not maintained. Instructed to replace tile with easily cleanable surface.
Counter and storage cabinets in employees break room/gift shop, can opener and interior panel of ice machine not cleaned. Instructed to detail clean and santize.
Ventilation not provided in employee men's and women's toilet room/locker room. Instructed to provide proper ventilation for each area. Utility sink does not have a visible backflow prevention device. Instructed to install backflow device on unit.

All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST RECAULK AROUND THE HANDSINK IN THE DISHWASHING AREA. CAULKING WORN OUT.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOORS NEEDS DETAIL CLEANING IN THE FOLLOWING AREAS: UNDER THE SHELVING INSIDE THE WALK IN COOLER, WALK IN FREEZER, UNDER THE ONE COMPARTMENT SINK IN THE GARDEN TERRACE, BEHIND THE ICE MACHINE, UNDER THE DRAIN BOARD OF THE DISH MACHINE, BEHIND AND AROUND THE VENDING MACHINE IN THE EMPLOYEE'S LUNCH ROOM.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. RUBBER GASKET OF THE WARMER, ALONG THE DOOR WORN OUT. INSTRUCTED TO REPLACE AND MAINTAIN.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE LIGHT SHIELD ON THE BULB ABOVE THE COUNTER IN THE SMALL DINING ROOM, OR CHANGE LIGHT BULB TO SHATTERED PROOFED LIGHT BULB.
