Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. REPAIR LEAK FROM AIR CONDITIONING PIPE. MAINTAIN DRY FLOOR.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. REPLACE LARGE POTS WITH SMALLER, EASILY SANITIZED POTS.
The walls and ceilings shall be in good repair and easily cleaned. PROVIDE SHELVING FOR BOXES AND TUBS FOUND ON FLOOR THROUGHOUT PREMISES. DETAIL CLEAN WALLS, CEILING AND SKY LIGHT IN PREP AREA AND ALL CONNECTED TUBING TO REMOVE ACCUMULATED DUST.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. PROVIDE MISSING LIGHT SHIELDS ABOVE FOOD PREP AND FOOD STORAGE AREAS THROUGHOUT PREMISES.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED ABOUT 30 MOUSE DROPPING IN REAR LOCKER AND FOOD STORAGE AREA AND 10 IN FRONT SERVICE AREA NEXT TO SERVICE STATION. ALSO NOTED OVER 1/4' GAP ON LEFT SIDE OF FRONT DOOR AND OVER 1/2' GAP UNDER REAR PREP AREA DOOR. INSTRUCTED MANAGER TO CONTACT PEST CONTROL SERVICE, CLEAN AND SANITIZE EFFECT AREAS AND CONTROL ACTIVITY. ALSO INSTRUCTED MANAGER TO SEAL ALL GAPS IN AND UNDER FRONT AND REAR DOORS.

FOUND FRONT AND EXIT DOOR NOT RODENT/ INSECTS PROOFED WITH OVER AN INCH GAP ON THE BOTTOM OF THE DOOR IN THE EXIT DOOR AND 1.4 OF AN INCH GAP ON THE BOTTOM OF THE DOOR IN THE FRONT ENTRANCE DOOR. INSTRUCTED TO RODENT/INSECTS PROOF THE DOORS. CITATION ISSUED SERIOUS.
FOUND GREASE CONTAINER OUTSIDE WITH HEAVY GREASE BUILT UP ON THE LID AND RIM AROUND THE CONTAINER. INSTRUCTED TO CLEAN IN DETAIL AND MAINTAIN. MANAGER ASKED ONE OF THE EMPLOYEE TO CLEAN THE CONTAINER LID AND RIM WHILE I WAS TYPING THE REPORT. GREASE CONTAINER IS NOW CLEANED AND MAINTAIN. CITATION ISSUED SERIOUS.
OBSERVED ONE OF THE EMPLOYEE RINSING THE POT AND PANS WITHOUT WASHING IT WITH SOAP AND WATER AND NOT SANITIZING IT. AFTER RINSING THE POT AND PANS HE PUT IT ON THE DRAINBOARD RIGHT AWAY. INSTRUCTED THE MANAGER HOW TO PROPERLY WASH, RINSE AND SANITIZE THE POTS AND PANS, AND INSTRUCTED HOW TO SET UP THE 3 COMPT.CITATION ISSUED CRITICAL.
