REAR EXPOSED HAND SINK UNABLE TO TURN OFF HOT WATER, MUST REPAIR.
All food not stored in the original container shall be stored in properly labeled containers. ALL PREPARED FOODS IN WALK IN COOLER AND FREEZERS MUST BE LABELED AND DATED. ALL BULK FOODS MUST BE LABELED.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. FRONT EXPOSED HAND SINK NOZZLES WITH ENCRUSTED FOOD DEBRIS, MUST CLEAN AND MAINTAIN. BULK CORN STARCH CONTAINER IN BASEMENT DIRTY, MUST CLEAN.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INTERIOR ICE MACHINE WITH RUST, MUST REMOVE.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOOR BASEBOARD UNDER FRONT EXPOSED HAND SINK DIRTY, WITH FOOD SPLATTER, MUST CLEAN. All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. MUST PROVIDE IN WALK IN COOLER.

Adequate and convenient hand washing facilities shall be provided for all employees. OBSERVED DURING INSPECTION NO SOAP OR PAPER TOWEL AT DESIGNATED EXPOSED HAND SINK. ALSO, SAME SINK WITH DIRTY PAN INSIDE AND LATER IN THE INSPECTION WITNESSED EMPLOYEE USING HAND SINK FOR FOOD PREP FILLING A FOOD CONTAINER WITH FOOD IN IT WITH WATER. DISCUSSED PROPER HAND WASHING AND PROPER USE OF EXPOSED HAND SINK WITH MANAGEMENT.
A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. NO CITY OF CHICAGO CERTIFIED MANAGER ON SITE WHILE FOODS BEING PREPARED AND SERVED. MANAGER ARRIVED APPROXIMATELY 15 MINUTES DURING INITIAL INSPECTION.
A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority.
