All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. REMOVE ALL THE TIN FOIL FROM THE PIPES AND GREASE TRAP UNDERNEATH THE 3 COMPARTMENT SINK.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CLEAN THE FLOORS IN THE BACK STORE ROOM AND IN THE BASEMENT HAS BUILDUP OF DUST AND DIRT ON THEM. SOME TILES IN THE STORE ARE IN POOR REPAIR AND SOME ARE MISSING. INSTRUCTED TO REPLACE SAME.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. PROVIDE A UTILITY / MOP SINK WITH HOT AND COLD RUNNING WATER UNDER CITY PRESSURE TO THE PREMISES. NOTE-MOP SINK IN STORE ROOM NOT CONNECTED TO ANYTHING.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. The floor behind the kitchen counter is dirty and the baseboards around the kitchen and back room perimeter have expanding foam sticking out of gaps, trim, reseal and clean the floors and baseboards so they are smooth and sanitary.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. The basement is cluttered with old boxes, clean the basement floor.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. The glass bulbs above the food prep area have no shields, install light shields.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. The long set of reach in grocery coolers and can opener is dirty; wash, rinse and sanitize the can opener and cooler in detail.

THE WALL BEHIND THE GRILL IS ROUGH WOOD, SMOOTH OUT AND SEAL THE WOOD SO IT CAN BE CLEANED EASILY.
THE BULB ABOVE THE ICE MACHINE HAS NO SHIELD AND THE END CAP IS MISSING OFF OF THE EAST BULB ENDS.
THE KNIFE HOLDER IS DIRTY; WASH, RINSE AND SANITIZE THE KNIFE HOLDER.
A SMALL HOLE WAS FOUND IN THE FLOOR BENEATH THE FIRE SUPRESSION SWITCH, REPAIR SAID HOLE AND SWEEP UP SPILLED DRY HERBS IN THE DRY STORE ROOM.
THE BATHROOM DOOR IS NOT SELF CLOSING, INSTALL A SELF CLOSING DEVICE.
PROPERLY LABEL AND DATE OPEN FOODS STORED ON PREMISES.
