All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines.OBSERVED EVIDENCE OF LIVE FRUIT FLY ACTIVITY MORE THAN 15 FLYING AROUND PREMISES ALSO REAR GARAGE DOOR & MAN DOOR NOT TIGHT FITTING ALONG BOTTOM.NO PEST CONTROL LOG BOOK ON SITE TO VIEW.MUST PROVIDE A PEST CONTROL LOG BOOK,DOORS NEEDS TO BE TIGHT FITTING,HAVE PEST CONTROL SERVICE PREMISES FOR FLY ACTIVITY.

LABEL AND DATE THE FOOD IN THE REACHIN FREEZER.
FOUND RUSTY RACKS IN THE REAR STORAGE AREA. INSTRUCTED TO REMOVE THE RUST AND PAINT THEM.
CLEAN THE FLOOR IN THE BASEMENT ALONG THE WALLS AND IN THE CORNERS HAS DUST AND DIRT ON THEM.
CLEAN THE FILTERS IN THE FRONT AREA HAS SOME GREASE BUILDUP. INSTRUCTED TO CLEAN.
CLEAN ALL VENTS THROUGHOUT PREMISES HAS DUST BUILDUP.
