found/observed live roaches 7 in kitchen area, dishwashing areas (1st fl., and bsmt.), also observed side entrance door(east)not roden proofed, door had 1/4 inch gap on bottom of door.citation issued#H000062821 with a court date of 10/28/2010, at 400 w superior in room #112 at 9:00am. or use pre-paid option.
found some foods not properly dated/labeled in coolers, freezers, must date and label all food items in coolers, freezers.
cooler shelving that had peeling paint, rust shall be repair/replaced.
non food cotact surfaces of cooking equipment had excess grease build up needs detailed cleaning(crevices), cooler shelving, cabinet drawers, storage shelving(1st fl.,bsmt. need detailed cleaning(crevices), prep tables need detailed cleaning.
floors under heavy equipment, cooking equipment in kitchen had excess grease, debris build up needs detailed cleaning(corners). floors in bsmt., prep areas, storage areas, storage rooms not clean need detailed cleaning(corners).
walls that had gaps, damaged, holes(around pipes), shall be repair,sealed. walls, ceilings that had peeling paint, grease build up need repairing, cleaning.
missing light shields in bsmt prep areas, storage areas, and display cooler shall be provided.light shields in prep area need cleaning(1st floor).
