REPAIR DAMAGED FLOOR AT ENTRANCE OF WALK IN COOLER IN FOOD STORE.
All food not stored in the original container shall be stored in properly labeled containers. MUST LABEL ALL UNLABELED MEATS IN STORAGE.
All dishwashing machines must be of a type that complies with all requirements of the plumbing section of the Municipal Code of Chicago and Rules and Regulation of the Board of Health. MUST WASH HANDS AFTER ANY POSSIBLE CONTAMINATION AND PRIOR TO PUTTING-ON FOOD GLOVES. POOR HYGIENIC PRACTICES, CRITICAL VIOLATION: 7-38-010(A) HOOOO77578-18.

REMOVE AND REPLACE RUSTY BROKEN METAL FLOOR INSIDE THE WALK-IN COOLER IN SELLING AREA, WERE MILK,EGGS ETC ARE STORED. SWEEP FLOOR BEHIND COOLERS AND UNDER SHELF. CLEAN FLOORS THROUGH-OUT BASEMENT, INCLUDING UNDER SHELVES AND ALONG WALLS. REMOVE ALL LITTER AND DEBRIS AND MAINTAIN.
NEED TO REPLACE BROKEN AND RUSTY METAL SHEETS BEHIND THE THREE COMPARTMENT SINK, BOTTOM PART. ADDTIONAL WEATHER STRIPPING IS NEEDED ON BOTH FRONT DOORS. REPLACE BROKEN WALL UNDER THE EXPOSED HAND SINK INSIDE THE CABINET UNDER THE EXPOSED HAND SINK. REPAIR AND PAINT WALL BEHIND THE COOLERS AT SOUTH WALL IN SELLING AREA. CLEAN FAN LOCATED ON TOP THE REAR DOOR IN PREP AREA, EXTREMELY GREASY. ALSO REMOVE DUST BUILD-UP FROM ALL CEILING AND WALL FANS/VENT COVERS. REPLACE BROKEN CAULKING FROM BEHIND THE THREE COMPARTMENT SINK(BLACK SLIME BUILD-UP).
REPLACE BROKEN LIGHT SHIELD IN BUTCHER AND PREP AREA, ALSO UNDER THE PRODUCE CASE WERE OPENED PRODUCE ARE STORED. PROVIDE END CAPS FOR THE LIGHT SHIELDS. REPLACE DEAD LIGHT BULBS IN PREP AREA.
PROVIDE SELF CLOSING DOOR FOR THE STAFF WASHROOM BY THE BUTCHER'S AREA.
REPAIR LEAKING PIPE UNDER THE WASH BOWL, AT FIRST FLOOR WASHROOM. REPAIR AND MAINTAIN LEAKS FROM PIPE NEAR CEILING IN BASMENT AND FROM VALVE ON PIPE IN REAR AREA BY OFFICE (6701 N. CLARK SIDE).
REMOVE UNNECESSARY ARTICLES FROM THE STORAGE ABOVE THE DOOR BETWEEN THE PREP AREA AND BUTCHER'S AREA. ORGANIZE ITEMS NEEDED. CLEAN AND ORGANIZE BASEMENT AREAS, REMOVE ALL UNUSED ITEMS AND STORE ALL ELSE NEATLY TO FACILITATE CLEANING (ELEVATED) AND TO PREVENT PEST HARBORAGE.
