OBSERVED EXCESSIVE ICE BUILDUP INSIDE OF THE 2 DOOR FREEZER NEXT TO THE ICE MACHINE. MUST DEFROST AND MAINTAIN.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN THE INTERIOR OF THE ICE MACHINE.
Smoking shall be prohibited in all public places except in designated smoking areas. OBSERVED VIOLATION OF CLEAN AIR ORDINANCE SECTION 7-32-020. OBSERVED SEVERAL ASHTRAYS WITH CIGARETTE BUTTS IN THEM IN THE BASEMENT DINING/BREAK ROOM AREA. MANAGEMENT WAS INSTRUCTED TO REMOVE THE ASHTRAYS FROM INSIDE OF THE BUILDING AND TO NO LONGER ALLOW RESIDENTS TO SMOKE INSIDE. MANAGEMENT INSTRUCTED TO POST SIGNS AT FRONT AND REAR ENTRANCES THAT SMOKING IS ALLOWED OUTSIDE AT LEAST 15 FEET AWAY FROM ALL DOORS, WINDOWS AND VENTILATION INTAKES. MANAGEMENT ALSO INSTRUCTED TO POST A NO SMOKING SIGN ON THE DOOR THAT WAS PREVIOUSLY USED AS A SMOKING ROOM FOR RESIDENTS. CITATION ISSUED FOR CLEAN AIR ORDINANCE VIOLATION 7-32-020.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.OBSERVED STAGNATE WATER BETWEEN FLOOR TILES IN PREP AREA(IN FRONT OF BOTH 3 COMPARTMENT SINKS,IN BOTH DISH WASHING AREA,AND WERE IS NEEDED IN KITCHEN. CLEAN FLOOR UNDER THE DISH MACHINE,DEBRIS SPECIALLY CORNERS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. REPAIR LEAKING PIPE UNDER THE 3 COMPARTMENT SINK(SOUTH SIDE)OF THE FIRST COMPARTMENT BY THE WALL.ALSO REPAIR LEAKING FAUCETS AT SAID SINK.REPAIR COLD WATER HANDLE AND SAID SINK(UNABLE TO TURN THE HANDLE).
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. DETAIL CLEAN ALL LIGHT SHIELD IN KITCHEN(DEAD INSECTS AND DUST BUILD-UP). ALSO CHANGE DEAD LIGHT BULBS, REAR PREP AREA.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.SEAL THE BOTTOM AND SIDE OF WALK-IN COOLER, IN KITCHEN
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. REPLACE BROKEN LIDS FROM OUTSIDE GARBAGE DUMPSTER.LIDS ARE BROKEN FROM THE HINGES, HOWEVER ABLE TO CLOSE.
