OBSERVED EMPLOYEE WASHING AND RINSING MULTI USE UTENSILS IN THE THREE COMPARTMENT SINK. PERSONALLY SHOWED HOW TO SET UP SINK: WASH RINSE AND SANITIZE 100PPM. CRITICAL VIOLATION:7-38-030 OUTER OPENINGS NOT PROTECTED. OBSERVED BOTH REAR DOORS WIDE OPENED WITHOUT A SCREEN DOORS. OBSERVED HOUSE FLIES(8 OR MORE )IN REAR PREP, KITCHEN, DISH ROOM,FRONT SERVING AREA AND BASEMENT. INSTRUCTED TO REMOVE FLIES AND PROVIDE A TIGHT FITTED SCREEN DOORS TO MAIN DOORS OR KEEP DOORS CLOSED AT ALL TIMES. SERIOUS VIOLATION:7-38-020 NEED TO LABEL AND DATED FOOD STORED IN COOLERS AND FREEZERS.ALSO WHITE CONTAINER WERE DRY FOOD STORAGE IS STORED.
