Found observed front entrance/exit door(south door)not rodent,and insect proofed, door had 1/4, to 1/2 inch gaps on bottom of door, and side of door, also found drive thru window not insect proofed, window had 1/4 inch gap on bootom of window. Found observed 2 compartment sink(prep sink), not draining properly, clear water backing up from floor drain, also found mop sink faucet in state of disrepair(broken)at time of inspection on 6/10/2011. Non food contact surfaces of walk in cooler shelving/rack, cooler shelving, storage shelving, cooking equipment not clean need detailed cleaning(crevices).
Floors under heavy equipment, cooking equipment, storage shelving, rear storage areas not clean need detailed cleaning(corners). floors in kitchen prep areas need re-grouting, floor had deep gaps.
Walls, ceilings in prep areas, storage areas need cleaning.
Light shields in kitchen areas need cleaning.
Exhaust canopy not clean need cleaning, exhaust vents(ventilation)in coolers need cleaning.
