No Certified Food Manager on premises when potentially hazardous foods are being served and prepared-soup at 160.5 F on steamtable. Instructed that a Certified Food Manager must be on premises at all times of operation. Serious citation issued 7-38-012.
Foods not protected during storage, preparation, display and service. Observed several cooked pasta's in prep cooler being covered with wet white linen napkins that has dirt stains, dingy and grey in color from not being cleaned. Instructed manager that all foods must be protected at all times and must remove linen napkins from foods. Serious citation issued 7-38-005(A).
Evidence of rodent activity found on premises. Observed rat dropping behind counter in bar area-approximately 10 dropping and in right corner approximately 5 droppings. Instructed to remove all evidence and detail clean areas. Serious citation issued 7-38-020.
Handsink for employees not maintained, installed or operated. Exposed handsink not connected-while running water in sink, water discarding from drain pipe on to floor, food handlers not able to properly wash hands. Instructed to use handsink in toilet room until exposed handsink is repaired and in good condition. Serious citation issued 7-38-030.
Sanitizing rinse for low temp dish machine not at proper concentration. Low temp chlorine santizer dish machine being use at time of inspection, not at proper santizing concentration. Chlorine santizer at 0ppm. Instructed manager that chlorine santizer must be at 100ppm for proper santizing of equipment and utensils. Critical sanitize issued 7-38-030. 'Held for Inspection' tag placed on dishmachine.
Toilet room doors not provided with self closing devices. Instructed to install self closing device on all toilet room doors.
Ventilation not provided in employees toilet room. Instructed to install exhaust fan or have window open at all times. Soda sprayers in bar area not properly installed. Instructed to install drainlines on holder to dispose of liquid waste in holders.
Floors not cleaned or maintained throught out prep and bar area. Instructed to detail clean floors in prep and bar along wallbase and reseal floor in bar area to have smooth and non arborsant surface.
Ceiling and walls not maintained in prep area. Instructed to replace all worn/damaged ceiling tile and repair reseal stainless steel panel around exposed handsink wall. Light covers not cleaned in prep area. Instructed to detail clean all covers.
Light bulbs not shielded in prep area. Instructed to sheild bulbs and install end caps on bulbs.
Multi use utensils not properly stored. Instructed to invert all cleaned utensils to prevent contamination before use.
Cutting baords attached to prep table not maintained. Instructed to replace all worn cutting boards. Ice bins not provided with covers. Instructed that ice must be protected during storage and use to prevent contamination.
Exterior cooking equipment, soda sprayer holders, sinks, prep tables, coolers, storage shelves and exhaust hood not cleaned. Instructed to detail clean and sanitize daily.
