OBSERVED AIR TEMPERATURE INSIDE OF THE 3 SUSHI COOLERS AT 60F. MANAGEMENT INSTRUCTED TO MAINTAIN BELOW 40F. CRITICAL VIOLATION 7-38-005A.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. THE FRONT DOORS ARE NOT RODENT PROOF. OBSERVED A 1/4' GAP ALONG THE BOTTOM OF THE DOUBLE DOORS. MANAGEMENT INSTRUCTED TO RODENT PROOF THE DOORS. SERIOUS VIOLATION 7-38-020.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN THE CONSTRUCTION DUST FROM HORIZONTAL SURFACES.

(CHICKEN-8 LBS; FISH-2 LBS; BEEF-5 LBS; PORK-10 LBS AT A TOTAL COST OF $110.00). INSTRUCTED TO PROPERLY DISPOSE OF PRODUCTS. MACHINE TAGGED AT THIS TIME UNTIL ALL THE REQUIREMENTS ARE MET WITH HACCP PROCEDURE. All food shall be from sources approved by health authorities and safe for human consumption.
FOUND FLOOR DRAIN UNDER THE DISHMACHINE WITH GRIME/DIRT-MUST KEEP CLEAN; MUST KEEP FLOORS DRY AT THE DISHROOM & ICE MACHINE AREA. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.
ALL FOODHANDLERS & DISHROOM WORKERS MUST HAVE HAIR RESTRAINTS, ESPECIALLY WHEN WORKING AROUND OPEN FOOD. All employees shall be required to use effective hair restraints to confine hair.
EXTERIOR OF ALL GARBAGE BINS MUST BE KEPT CLEAN. All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.
WALLS OR CEILINGS AFFECTED BY SMALL/FRUIT FLY ACTIVITY MUST BE CLEANED & SANITIZED. DUSTY CEILING VENTS MUST BE MAINTAINED ALSO. The walls and ceilings shall be in good repair and easily cleaned.
