All food establishments shall be provided with an adequate supply of hot and cold water under pressure properly connected to the city water supply. NO HOT WATER AT FRONT THREE EXPOSED HAND SINKS INSTALLED WITH MIXING VALVES. MUST PROVIDE HOT WATER.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. REAR EXIT DOOR OF KITCHEN NOT RODENT PROOF WITH OPEN 1/2 INCH GAP ON BOTTOM OF DOOR. OPEN GAP ON BOTTOM DOOR ON FRONT SIDE EXIT AND OPEN GAP BETWEEN TWO FRONT MAIN DOORS. MUST SEAL ALL AND MAKE TIGHT FITTING.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. DIRTY FLOOR DRAIN BEHIND BAR. MUST CLEAN. ALL STORED ITEMS THROUGHOUT 2ND FLOOR STORAGE MUST BE ELEVATED 6' OR ABOVE.
All food not stored in the original container shall be stored in properly labeled containers. ALL BULK FOOD CONTAINERS MUST BE LABELED.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. ALL CONSTRUCTION DEBRIS/MATERIAL MUST BE REMOVED FROM REAR PATIO AND THROUGHOUT BASEMENT.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST INSTALL A SPLASH GUARD BETWEEN REAR PREP EXPOSED HAND SINK AND STORAGE SHELVING.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. DIRTY TABLE TOP CAN OPENER. MUST CLEAN AND MAINTAIN AFTER EACH USE.

All cold food shall be stored at a temperature of 40F or less. APPROXIMATELY 5 LBS ASSORTED CHEESES FOUND AT 62.2F, 3 LBS CUT VEGETABLES AT 64.7F, 3 QTS SALAD DRESSING AT 64.9F. ALL FOODS DISCARDED. ALL FOODS FOUND IN LINE COOLER THAT WAS UNPLUGGED. MANAGEMENT PLUGGED IN COOLER AND LATER MAINTAINED TEMPERATURE OF 34.7F.
The walls and ceilings shall be in good repair and easily cleaned. WALL MOUNTED SOAP DISPENSERS (2) IN REAR DISH AND PREP WITH BROKEN HANDLES. MUST REPLACE. MISSING AND WATER DAMAGED CEILING TILES IN 2ND FLR. DRY STORAGE ROOM. MUST REPLACE.
The flow of air discharged from kitchen fans shall always be through a duct to a point above the roof line. ALL COOKING EQUIPMENT MUST BE PROPERLY USED UNDER VENTILATION.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. ALL UNNECESSARY CONSTRUCTION MATERIAL THROUGHOUT BASEMENT MUST BE REMOVED OR STORED ELEVATED AND ORGANIZED.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. CRACKED/BROKEN ICE SCOOP FOUND ON TOP OF ICE MACHINE. MANAGEMENT DISCARDED.
