All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. Observed over 20 live small flies on walls and ceiling tiles throughout dishmachine room. Instructed manager to remove/eliminate all insect activity. Advised to call pest contro operator for service.Citation issued. Ticket #H000057357.
A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. The following minor violations still exist from report #147488 dated 04/15/09 and must be corrected by reinspection(01/19/10)- #32 Must replace/repair all damaged reach in/walk in coolers rubber door gaskets #33 observed accumilated food debris on shelves thoughout prep area, accumilated food debris inside prep coolers rubber door gaskets and door hindges #35 Must detail clean walls thoughout prep, dishwash and dry storage areas also must replace all unclean ceiling tiles throughout #38 1-compartment sink (by convec therm machine) pipe leaking. ICE SCOOPS MUST BE STORED IN A CLEAN PLACE. Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination.
Following plumbing issues must be addressed: missing hot water knob at dump sink (by ice machine in hallway); leakunder dishmachine and elbow pipe of hand sink in dishroom and under 2-comp sink in prep room.
All employees who handle food shall wash their hands as often as necessary to maintain a high degree of personal cleanliness and should conform to hygienic practices prescribed by the Board of Health. Observed food handler at sushi bar using bare hand contact (wasabi, raw fish) to plate these food items, then wiping his hands on a wiping cloth and rinsing this cloth at the prep sink after which he plated a different dih order without washing hands properly.
