All food establishments that display, prepare, or store potentially hazardous food shall have adequate refrigerated food storage facilities. THE DELI DISPLAY HAS AN AMBIENT AIR TEMPEWRATURE OF 63 F. COOLER TAGGED HELD FOR INSPECTION. Fax a tag removal request to (312) 746-4240 when the equipment is ready for re-inspection.
All cold food shall be stored at a temperature of 40F or less. APPROXIMATLY 15 POUNDS OF RAW EGGS, MILK AND COLD CUTS WERE HELD AT 50 TO 60 F. $25.00 WORTH OF PRODUCT WAS DUMPED AND DENATURED.
THE 3-COMPARTMENT SINK IS DIRTY, WAS TOOTHBRUSHES AND SHAVING SUPPLIES ON THE SINK AND NO STOPPERS FOR THE 3-COMPARTMENT SINK. THE SINK WAS CLEANED AND THE STOPPERS WERE PURCHASED AND BROUGHT TO THE STORE.
Clean and degrease the equipment and surfaces in detail. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Wash, rinse and sanitize all of the counters front and rear. Clean and maintain the knife holder in sanitary condition. Dust the store shelves and maintain cleanliness.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. All utensils shall be thoroughly cleaned and sanitized after each usage.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Clean and maintain the floors and baseboards so they are smooth and sanitary from corner to corner.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. INSTALL SHEILDS IN THE KITCHEN.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. Repair the self closing door.

All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines.WE OBSERVED APPROXIMATELY 25 RODENT MICE DROPPINGS THROUGHOUT CLUTTERED REAR ROOM.MUST DETAIL CLEAN,SANITIZE ALL AFFECTED AREAS,REMOVE CLUTTERED ITEMS,PEST CONTROL CO SERVICE NOW RECCOMENDED,ALSO MUST SEAL ALL NOTICEABLE OPENINGS THROUGHOUT WERE FLOOR MEETS WALL AREAS.
Adequate and convenient toilet facilities shall be provided. They should be properly designed, maintained, and accessible to employees at all times.WE OBSERVED EMPLOYEES TOILET ROOM NOT MAINTAINED LACKING SOAP,HAND PAPER TOWELS DURING THIS INSPECTION.
The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance.WE OBSERVED OUTSIDE GARBAGE DUMPSTERS NOT MAINTAINED THAT IS LIDS NOT CLOSING PROPERLY OVERFLOW OF GARBAGE,HIGH WEEDS ALL AROUND GROUND IN SAID AREA.
All dishwashing machines must be of a type that complies with all requirements of the plumbing section of the Municipal Code of Chicago and Rules and Regulation of the Board of Health.WE OBSERVED DRAIN PIPE IN POOR REPAIR ATTACHED TO 3 COMP SINK OF GREASE TRAP WATER LEAKING-MUST REPAIR TO GOOD WORKING CONDITION PER THE CITY OF CHICAGO MUNICIPAL CODE.
Smoking shall be prohibited in all public places except in designated smoking areas.WE OBSERVED EVIDENCE OF SMOKING INSIDE THE PREMISES I.E.BOTTLE CAP USED AS AN ASHTRAY WAS OBSERVED ON A TABLE BEHIND FRONT SERVICE COUNTER.ADVISED MANAGER NO SMOKING INDOORS OR WITHIN 15 FEET OF DOOR WAYS. 7-32-020(C).
