BULK FOOD CONTAINERS NEED LABELS-MUST PROVIDE. All food not stored in the original container shall be stored in properly labeled containers. INTERIOR OF PANELS OF ICE MACHINE AT 5TH FLOOR, CURTAINS OF DISHMACHINE AT 6TH FLOOR NEED CLEANING-MUST MAINTAIN. All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. FOLLOWING NEED CLEANING: FLOORS ALONG WALLBASES UNDER DISHMACHINE, BEHIND ICE MACHINE, AROUND GREASE TRAP AT 5TH FLOOR KITCHEN; UNDER WALK-IN COOLER SHELVES AT 6TH FLOOR. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FAN GUARDS/COMPRESOORS OF COOLERS & EXHAUST VENTS ABOVE THE DISHMACHINES MUST BE KEPT CLEAN & MAINTAINED. The walls and ceilings shall be in good repair and easily cleaned. CORRECTED CORRECTED FOLLOWING PLUMBING ISSUES MUST BE ADDRESSED: LEAK AT BOTTOM PIPE OF DISHMACHINE SPRAYER AREA (5TH & 6TH FLOORS); HANDSINK FAUCET LEAK AT MAIN KITCHEN. THE SPRAYER HOOK AT 3-COMP SINK IS MISSING-MUST REPLACE (6TH FLOOR). Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. THE GREASE TRAPS AT 5TH & 6TH FLOORS VERY RUSTY & SHOWING SIGNS OF WEAR & TEAR AT THE BASES, INSTRUCTED TO RE-PAINT OR REPLACE. EVIDENCE OF MILDEW & WORN-OUT CAULKING AT SPLASH GUARDS OP DISHMACHINES-MUST REPLACE.
