A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. NO CITY OF CHICAGO SANITATION CERTIFICATE POSTED ON PREMISES.
All food not stored in the original container shall be stored in properly labeled containers. ALL BULK FOOD CONTAINERS MUST BE LABELED. The licensee shall inform the public in writing of the Chicago Department of Public Health?s Consumer Advisory relating to consuming raw and undercooked animal products.MUST BE POSTED.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. VENTILATION IN LADIES AND EMPLOYEE WASHROOM NOT OPERATIONAL, MUST REPAIR. ALL VENTS IN ALL WASHROOMS DIRTY, MUST CLEAN. ALL GUN HOLDERS BEHIND BOTH BARS MUST PROPERLY DRAIN TO FLOOR DRAINS.

MISSING COVING/BASEBOARD ALONG FLOOR UNDER KITCHEN THREE COMPARTMENT SINK. FLOOR UNDER BAG IN BOX SODA DISPENSE SYSTEM IN OUTDOOR STORAGE WITH SPILLAGE, MUST CLEAN.
The walls and ceilings shall be in good repair and easily cleaned. WATER DAMAGED CEILING TILES ABOVE COOKS LINE, MUST REPLACE.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. DIRTY/STAINED LIGHT SHIELDS ABOVE COOKS LINE, MUST CLEAN.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. BLACKENED PLASTIC DISH RACKS MUST REPLACE.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. MUST REMOVE OUTDOOR GAS BBQ GRILL FROM PREMISES, AND ALL UNNECESSARY ITEMS THROUGHOUT BASEMENT MUST BE REMOVED. ALL OTHERS STORED ORGANIZED AND ELEVATED FROM FLOOR.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. BASEMENT EMPLOYEE WASHROOM DOOR MUST HAVE SELF-CLOSING DEVICE.
