All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. REAR EXIT DOOR FOUND WITH LARGE 2' GAP ON BOTTOM OF DOOR, NOT RODENT PROOFED. INSTRUCTED TO PROVIDE TIGHT FITTING SEAL ON BOTTOM OF DOOR.
A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. NO CITY OF CHICAGO CERTIFIED MANAGER PRESENT DURING INSPECTION. INSTRUCTED MUST HAVE CERTIFIED MANAGER ON SITE AT ALL TIMES WHEN FOOD IS BEING PREPARED AND SERVED.
Smoking shall be prohibited in all public places except in designated smoking areas. OBSERVED THREE ASHTRAYS, ONE WITH A CIGARETTE BUTT, ON TABLES IN OUT DOOR BEER GARDEN PATIO.
All food not stored in the original container shall be stored in properly labeled containers.ALL PREPARED FOODS IN WALK-IN COOLER AND BOX FREEZER MUST BE LABELED AND DATED.
The walls and ceilings shall be in good repair and easily cleaned. EXCESSIVE GREASE ON WALLS THROUGHOUT KITCHEN. MUST CLEAN AND MAINTAIN. MISSING WALL TILES AT ENTRANCE OF KITCHEN. MUST REPLACE. MUST INSTALL CEILING ABOVE BASEMENT ICE MACHINE.

CLEAN THE WALLS AROUND THE PREP AREA. REPAIR THE LOOSE WALL TILES NEXT TO THE GRILL.
OBSERVED THE DRAIN LINE FOR THE ICE BIN BEHIND THE BAR DRAINING WATER INTO A BUCKET. MUST REMOVE BUCKET AND DRAIN TO A FLOOR DRAIN.
