The shake machine is holding the ice cream mix in the top of the machine at 44-45F.

There is a splitter attached to the faucet at the mop sink, with one side going to a chemical dispenser and the other to a hose with a sprayer, and the water is left on. This forces the backflow device to stay open, so it will not work to protect your fresh water from contamination if water pressure reverses.
REQUIRED CORRECTION: A backflow or backsiphonage prevention device installed on the water supply system shall meet American Society of Sanitary Engineering standards.

Walk-in cooler holding food temperatures at 43-45F. (Suspect the right fan not blowing air properly, may be a contributing factor?)
(*Also noted that the cooler door seems to be loosing a little cold air at the bottom right side, and the gasket seems to be pulling off at the top - make any needed maintenance repairs also).

Clean equipment and utensils shall be stored in a self-draining position that permits air drying and covered or inverted. Employee entering kitchen after returning from the restroom not observed to wash their hands again in the kitchen. Two other employees starting their shift were observed either doing only a single wash or washing for 5-10 seconds.
REQUIRED CORRECTION: CORRECT IMMEDIATELY. After defecating, contacting body fluids and discharges, or handling waste containing fecal matter, body fluids, or body discharges, and before beginning or returning to work, food employees shall wash their hands twice. After using the toilet room, hands must be washed first at a handwashing lavatory in the toilet facility and again at a handwashing lavatory in the food preparation area.
