Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
- Observed floors, walls, and/or ceilings that were not constructed of approved materials. Scrape and repaint the ceiling, flaking paint in numerous areas. Roof reportedly fixed, ceiling needs to be repaired ASAP.

The light intensity shall be at least ten foot candles (one hundred eight lux) at a distance of thirty inches (seventy-five centimeters) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning.
- Observed a build-up of dirt and debris. Entire kitchen needs to be cleaned from top to bottom. Start at one area and move from section to section. Buildup of food/grease on floor/equipment.
