  • Critical: fish in bowl on top of bagged lettuce. keep all raw food on bottom shelf. Observed food that was not properly protected from contamination by separation, packaging, and segregation.
    To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
  • Non-food contact surfaces of equipment are dirty. Clean sides of fryers and other equipment in kitchen.
    Nonfood-contact surfaces of equipment shall be kept clean.
  • Observed floors, walls, and/or ceilings that were not constructed of approved materials. Repair floor in walk in cooler.
    Where FSO or RFE activities are conducted, materials for indoor floor, wall, and ceiling surfaces shall be smooth, durable, and easily cleanable
  • Repair the supply of electricity in the walk in cooler. The light intensity in a customer self-service area, inside equipment, toilet room, or areas used for hand washing or equipment or utensil storage was less than twenty foot candles.
    The light intensity shall be at least twenty foot candles (two hundred fifteen lux): at a surface where food is provided for customer self-service such as buffets or salad bars or where fresh produce or packaged foods are sold or offered for human consumption

  • Equipment and/or components are not maintained in good working order. Clean floor in walk in cooler, repair/replace floor in walk in cooler where needed to provide a smooth and cleanable surface.
    Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
  • Observed floors, walls, and/or ceilings that were not constructed of approved materials. Scrape and repaint the ceiling, flaking paint in numerous areas. Roof reportedly fixed, ceiling needs to be repaired ASAP.
    Where FSO or RFE activities are conducted, materials for indoor floor, wall, and ceiling surfaces shall be smooth, durable, and easily cleanable
  • Observed floors, walls, and/or ceilings that were not constructed of approved materials. Kitchen floor needs to have broken/missing floor tile replaced, so the floor can be smooth and cleanable.
    Where FSO or RFE activities are conducted, materials for indoor floor, wall, and ceiling surfaces shall be smooth, durable, and easily cleanable
  • Observed no towels or hand drying device at the handwashing sink(s). Provide soap and hand towels at kitchen hand sink.
    To prevent contamination by hands, each handwashing sink or group of adjacent handwashing sinks shall beprovided with: Individual, disposable towels
  • The light intensity in the walk-in refrigerator, dry food storage area, or other area during cleaning was less than ten foot candles. REPLACE BULB.
    The light intensity shall be at least ten foot candles (one hundred eight lux) at a distance of thirty inches (seventy-five centimeters) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning.
  • Observed a build-up of dirt and debris. Entire kitchen needs to be cleaned from top to bottom. Start at one area and move from section to section. Buildup of food/grease on floor/equipment.
    The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.

  • The warewashing equipment and/or components were not cleaned at the required frequency. Clean interior and exterior of doors of dishwasher
    A warewashing machine, the compartments of sinks, basins, or other receptacles used for washing and rinsing equipment, utensils, or raw foods, or laundering wiping cloths
  • Non-food contact surfaces of equipment are dirty. Clean racks in walk in cooler to remove buildup, floor also.
    Nonfood-contact surfaces of equipment shall be kept clean.
  • Critical: The wastewater is backing up under the 2 bay sink/grease trap in kitchen, call a plumber asap to clean sewer.
    Sewage shall be conveyed to the point of disposal through an approved sanitary sewage system or other system, including use of sewage transport vehicles, waste retention tanks, pumps, pipes, hoses, and connections that are constructed, maintained, and operated according to law.
