  There is no consumer advisory notice on the menu regarding the health risks associated with the consumption of raw or undercooked animal foods. The notice will be on the newer menus which were not available in hard copy. I provided the person in charge with a copy of the DC DOH consumer advisory placard. (Corrected On Site)

  Grease and dust residue are on the conduit connected to the ventilation hood and the wall above the rear kitchen entrance.

  There is no light source inside the reach-in freezer. Blown bulb (no lighting) in the reach-in refrigerator.
