The facility could not provide proper documentation for parasite destruction for the raw and undercooked fish products. Raw chicken was stored on top of ready to eat beef gravy in the walk-in refrigerator. Several fruit flies were present in a bottle of vermouth at the bar.
The facility did not have a consumer advisory for the menu items that could be served raw or undercooked.
An employee was observed filling a bottle with water at the handsink across from the dish machine area.
Employee beverages were present on the cook line cold top cutting board and preparation table.
A spray bottle of water in the back preparation area was not labeled as to its contents.
The facility lacked a food preparation sink.
Shelves in several reach-in refrigerators throughout the facility were chipped and rusted. A cloth towel was used to cover lettuce in the cook line cold top refrigerator.
The door gaskets on the two door cook line reach-in refrigerator, back bar reach-in refrigerator and cook line cold top refrigerator were torn. The caulking in the cook line hood was peeling and in poor repair. The back area preparation table was not adequately sealed to the wall. A lid to a container of rice in the back preparation area was cracked. Shelves in the walk-in refrigerator were rusted.
The dish machine drainboard was not adequately sealed to the wall.
The cook line cutting boards were stained. Mold was present in the ice macine located in the closet in the back bar area.
Refrigeration door gaskets throughout the facility were soiled with food debris. The cook line hood was soiled with dust and grease.
Clean containers were stacked wet on a shelf in the back preparation area.
A spatula was stored in a crevice between the back area preparation table and the wall.
Several single use plastic containers were being reused to store various foods throughout the facility.
The warewashing area handsink and the back bar handsink were not adequately sealed to their respective walls.
The floor throughout the kitchen was in poor repair. Ceiling vents on the cook line were soiled with dust. The wall behind the equipment in the back preparation area was soiled with food residue and grease. The air intake filters above the main ice machine were dusty. Several vents throughout the facility were dusty.
A light in the employee restroom was not operational.
Employees involved in food preparation were wearing a watch and a bracelet.
