Violation descriptions and comments

Raw chicken was stored above ready to eat coleslaw in the walk-in refrigerator. Raw pork was stored next to ready to eat pineapple in the bar walk-in refrigerator. Several fruit flies were present in a bottle of Vermouth at the bar.
An unapproved employee beverage was stored next to clean dishes on the cook line.
Facility employees could not demonstrate knowledge regarding reheating temperatures, hot-holding temperatures or proper cooling methods.
Refried beans were 126�°F in the cook line hot-holding unit.
Salsa mix was 51�°F in the wait station cold-top refrigerator. Pork was 46�°F in the cook line cold-top refrigerator. Shredded cheese was 46�°F in the cold-top refrigerator across from the cook line stove.
A Y valve with stops was located downstream of the atmospheric vacuum breaker without the proper back flow prevention device.
Radio wires and personal phone wires were observed stretched across and in the basin of the dish machine area hand sink, blocking access to the sink.
No paper towels were present at the bar hand sink.
As stated by an employee, a chemical spray bottle of disinfectant was being used on food contact surfaces on the back preparation tables.
Salsas in the front wait station holding unit was uncovered.. Three containers of tamales were cooling covered in the walk-in refrigerator, as opposed to cooling uncovered. Beans and several other foods on the cook line hot-holding unit were uncovered. Lemons in server station in the dining area were stored uncovered.
Several cutting boards in the front wait station and on the cook line were scored.
The door gasket on the front wait station reach-in freezer was torn. The door gasket on the front wait station reach-in refrigerator was torn. The caulking on the three compartment sink and the cook line hood was torn. Several door gaskets on the cook line reach-in refrigerators were torn. Several plastic containers on the cook line cold-top refrigerator were chipped. The door gasket on the wine reach-in refrigerator at the bar was torn.
The caulking on the dish machine drain board was torn.
The caulking on the dish machine area hand sink was soiled with mold. Several door gaskets on the cook line reach-in refrigerators were soiled with food debris. The gasket on several cook line reach-in refrigerator drawers was soiled with food debris. Several door gaskets on the bar reach-in refrigerators was soiled with food debris.
Sanitizer in the bar area did not meet the minimum requirement of at least 100ppm of quaternary ammonia.
Several utensils in the front wait station and in the cook line was observed stored in 72�° of water.
Several re-used containers were storing spices in the back preparation area.
The caulking on all of the hand sinks in the dish machine area and next to the office were torn.
A light above the dry dish rack was not functioning. A light under the cook line ventilation hood was not functioning. A light in the cook line area was not functioning.
