- Critical. Violation: 17-09-1 Lack of use of chemical test kit when using chemical sanitizer at three-compartment sink/warewashing machine, On call back managr stated they did not have sanitizer test strips.
- Violation: 51-16-1 No plan review submitted and renovations in progress, gave operator website to get plan review packet and application advised 150.00 fee. On callback manager stated he was told plan review had been sent it. I called plan review spoke to George and he checked under license number, address and name and no plans or payment had not been received.
- Critical. Violation: 53B-08-1 No proof of required employee training provided. All public food service establishments must provide the division with proof of employee training upon request, including, but not limited to, at the time of any division inspection of the establishment. On call back inspection one employee over 60 days without proof of employee training, manager stated they do not have any aporoved training program.
