Exterior premises shall be well drained and maintained free of high grass, poisonous plants and pest harborages and breeding sites, nuisances and any other object or condition which may be hazardous to children. Hazardous supplies, tools and equipment, both indoors and outdoors, shall be inaccessible to children at all times.
Exterior premises are not free of nuisances or other hazards to children. There is an exposed screw tip on the equipment for learning geometric shapes. The playground equipment has adhering mildew.

- Child Care Worker Certificates
Day Care Ordinance 16-435, page 14 & 15. Every owner, employee and volunteer (except janitors) who provides care,control or supervision of children shall obtain a child care worker certificate within 30 days of starting work. A child care worker certificate shall be valid for two years. A copy of the child care worker certificate shall be maintained at the day care center for each owner, employee and volunteer who cares for, controls or supervised children at the facility and shall be available for review upon request by the Health authority. Child care worker certificates shall not be used for any other purpose and are not transferrable to any other person.
Individuals who provide care for, control or supervise children do not have current child care worker certificates. Individuals who provide care for, control or supervise children have not obtained child care worker certificates.
Corrective action: Every owner, employee and volunteer (except janitors) who provides care,control or supervision of children shall renew their child care worker certificate every two years. Corrections must be made within 30 days. - Lavatories
Day Care Ordinance 16-429C(4) & (7), page 8. All handwashing lavatories shall be provided with hot and cold water under pressure, tempered by means of a mixing valve or combination faucet. The hot water available to children shall not exceed 120 degrees F. Combination fixtures such as faucet/water fountains are prohibited. There shall be at least one handwashing lavatory in each restroom or immediately adjacent to each restroom. A handwashing lavatory must be installed in each room with a diaper changing station. Soap and a hand-drying device or disposable towels shall be provided for use by both children and staff at each lavatory.
Soap is not provided at lavatory in the 3 year old's room. There is not at least one handwashing lavatory in each restroom, immediately adjacent to each restroom or in each room with a diaper changing station. The lavatory in the 2 year old room is not accessible to the children.

Additionally, fish and seafood shall be refrigerated to a temperature of 34 °F or less.

- Food Establishment Permit/Food Handler Card
TFER 229.171(f) & (g) - Page 126; FW food ordinance Sec. 16-141 A person may not operate a food establishment without a valid health permit to operate. The permit is not valid without a valid Certificate of Occupancy issued by the City of Fort Worth Department of Development.
