Exactly the reason why big business (i.e. big insurance companies) and the wellbeing of people do not mix.

Keeping people alive is generally not cost-effective and profit-generating.

You can blame the "exempt" employee nonsense for non-hourly paid nurses on George W. Bush, NOT Obamacare. He was the one who changed some of the who is exempt vs. non-exempt rules when he was in office. Mostly in favor of the corporations, of course.

Actually the basic premise of ACA isn't bad, require everyone to have insurance and make various levels of plans available to them. It's something that we conservatives had wanted people to do for a long time. The biggest problems with ACA is that it's just so badly written (as in toss stuff into it and figure it all out later), is more of a giant group policy rather than an individual policy, and it's not ready for prime time (arbitrary roll out dates being the norm with the Government) .

Here's a nice clear definition of what the law says. The bad part is it's not adjusted for inflation. Some of the people who were non-exempt were making a killing.

It's funny that you mention the high malpractice rates and tort law in the same sentence as the AFA. Does it not seem strange that any mention of tort reform fell on deaf ears during the entire debate of the AFA? It was brought up on several occasions in the political arena but quickly dismissed. Why is that subject not on the table in any political discussions?

Going back to what I had written earlier, people used to pay for the doctor's visit and tests and submit for reimbursement. Doctors were paid, deductibles were gradually chipped away.

I saw exchange plans with $3,000 per person deductibles; how is paying a large deductible upfront affordable to a low-income person whose modest premium is being subsidized?

Well it really isn't. Let's face it, health care services, no matter if it's the current system or single payer, will always be expensive and it boils down to how it will be paid for. For many people the real costs were mostly hidden.

For the most part some employer plans did away with the deductible, and lets not forget that employer based health insurance was in order to attract and keep the best talent and workers. These days with high unemployment many employers no longer need a great health care plan to attract as much talent. So now we're going to see what some of this care costs.
