The card is one that I am actually making (sort of). It's based off of an episode of The Simpsons that he and I liked.
His hobbies- working on things around the house (he already has a million tools), making tacos (I thought about buying a personalized cutting board for him but it's a lot more $ than I thought it would be), skiing, snowboarding, watching TV, working out, and spending time with me LOL).
Honestly, he and I have pretty much the same hobbies. Aside from going out to eat and maybe going outside on the weekends on long walks, we really just are homebodies. We have meshed well together because we have the identical sense of humor and hate the same things (I know that's probably an awful thing to say but we bonded over things that disgust us!).
I'm really bad at gift giving especially on Valentine's Day (I only had 1 Valentine's Day where I wasn't single). This is the guy I have gone out on 6 dates with in just a bit over a week. He is gone this week for a skiing trip with friends but we have a date the day after he comes back. Again we have seen each other every other day or so.
He made a reservation at a nice restaurant. I figure he will just do something like flowers and chocolate which I like (I'm traditional). I don't want to give him something safe (like just giving him a cute mug with chocolates in it). I was thinking of putting together a bag of things I know he needs but it's really random (SPF 70 spray, Tend Skin for shaving, and a keychain from a place where we had 1 of the best dates of my life along with Hershey Hugs and Kisses). Does that seem good or too impersonal? I'm adding a card to it.

I think that it is a cool idea, especially since you are making the card and you are including both things he can actually use and some mementos. It doesn't seem impersonal at all.
I found a website for something called a "man crate." Basically it's a rustic gift box for a man filled with things like jerky, BBQ stuff, etc. it's kind of overpriced, but it might give you additional gift basket ideas (or if you want to spend the money and like the manly crate with crowbar idea, get the man crate).
For me whenever i get gifts for people, i like to get them something personal. i always to try to get them something that they will get a lot of uses out of them, something that fits their personality, and something that would make them remember me.
