Talked with the person in charge of the facility. Observed no evidence of pests at the time of the complaint inspection. Observed the last three pest control inspection reports from ORKIN the last date being 5/10/2010. There is no evidence of pests reported on the reports. ORKIN comes out to treat every month and treats for pests such as roaches and mice. I did tell the PIC to put screen doors on the doors which will be opened for cooling purposes. Also told the PIC to clean all non food contact surfaces very well including behind equipment, to prevent issues in the future.

At time of follow-up observed all critical violations abated, execept for the ice machine which still had mold in it. However, there was no ice in the machine and the facility is not using it to make ice. They have plans to buy a new machine. In the meantime they are using bagged ice.

Spoke to the PIC about the foodborne complaint. The facility has not had any sick employees or equipment failures from the time of the complaint. Facility knows about the complaints. Facility has not had anyone call in and say they had a foodborne illness since the complaint. Closing the complaint until further notice. Facility had a follow-up just a couple of weeks from the complaint date.

Filthy, dirty, gross. The restroom disgusting, looked like it hasn't been cleaned for weeks.
The smell in the restaurant is a combination of urine and mold. It made me sick.
The hostess Jeana should find an other job. Shes arrogant, rude and totally lack any customer service skills. She pretends she is a manager when dealing with customers. Maybe it would be a good to have her wear a name tag.
This place is a hazard to customer health and a rip off . They charge fine dining restaurant prices for garbage food, garbage atmosphere and GARBAGE CUSTOMER SERVICES.
If this place wont get closed soon I bet people will start getting realy sick.
