Arrived at facility to consult violations found on Standard inspection on 6-23-11.
Observed sewage water coming out of floor drain under espresso machine. Facility closed voluntarily.
Facility may not reopen until given approval from Health Authority.
Conditions that must be met until reopen can be granted:
1) Floor drain must be cleaned and sewage water no longer escaping onto floor.
2) Thoroughly clean and sanitize entire floor.
3) From previous inspection - pest control conducted by licensed pest control operator - retain receipt, please
4) Clean all mouse droppings from floor, shelves, and equipment

1) Sewage problem repaired
2)Floor clean and sanitized
3) Pest control operator on 5-27-11
4) Floor and shelving has been cleaned of mouse droppings

Critical violations corrected. Pest control operator at facility on 6-28-11. Continue pest control and good sanitation practices to limit, remove, and Prevent Pests.
Non-critical violations on this report are repeat violations. Please correct by next Standard inspection.
