Toxic materials shall be properly identified and stored in a manner that can not contaminate food.

The facility showed parasite destruction information from True World Foods and a Certificate of Compliance from Yamasho, Inc.
I contacted Yamasho and requested that a document be faxed that includes documentation of parasite destruction. The Certificate of Compliance indicated compliance of 21CFR 123, but did give any details on freezing for parasite destruction.
The violation will remain open until the issue has be clarified. Please contact Yamasho if necessary to obtain parasite destruction information.

III. Handwashing, Prevention of Contamination from Hands (+) Observed hand washing sinks being used for other purposes. Observed no hand washing sign posted at hand washing sinks at sushi bar, bar, and throughout kitchen. Observed no paper towels at hand washing sink behind bar.
Handwashing facilities are adequate, conveniently located and accessible for employees. Handwashing facilities are properly supplied. Hands are properly washed when required. Exposed ready-to-eat foods are not touched by bare hands.
V. Food from an Approved Source (-) Documentation of parasite destruction for fish products served not available at time of inspection.
All foods are from an approved source (including but no limited to milk and milk products, eggs, cheese, meat, exotic meats, poultry, fish, shellfish, wild mushrooms, baked goods, processed foods). Written documentation of parasite destruction is maintained for applicable fish products. Shellstock identification tags are maintained for 90 days.
VI. Thawing (+) Observed proper thawing of frozen shrimp in walk-in cooler.
Foods are thawed in accordance with Ohio Administrative Code.
VII. Cooking, Reheating, Cooling, Hot and Cold Holding (-) Observed raw shell eggs holding at 59F on counter top. Discussed proper cooling procedures.
