Failure to maintain buildings, physical facilities in repair sufficient to prevent food from becoming adulterated. Specifically, I observed water dripping from ceiling into and next to ingredient bin, paint chipping from beam over cooling rack. Ingredient bin was removed for cleaning at time of inspection.
- Maintenance of equip., utensils, and finished food packaging
Failure to maintain utensils in an acceptable condition through appropriate cleaning and sanitizing. Specifically, I observed a white food brush stored on a dirty air handling unit and a paddle with white end improperly leaning against a building beam. (white utensils are for food contact surfaces)

Failure to maintain equipment, used to convey food in a manner that protects against contamination. Specifically, I observed 2 worn or tattered food conveyance belts in production area before product was panned
- Thawed appropriately
Failure to thaw frozen raw materials in a manner that prevents them and other ingredients from becoming adulterated. Specifically, I observed 8 pallets or roughly 8,000lbs of shredded lite mozzarella cheese thawing at room temperature in production area. 2 temperatures were taken 1 at 28F and 1 at 52F
