- Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food, as specified under 2-201.11(A)
Observation:Food employees must be able identify their obligation to report illness to PIC
- Disclosure of menu items offered or served raw or undercooked
Observation:items that can be cooked less than well done (ie burgers) need to be denoted with an asterisk as well.
- Certified Food Protection Manager
Observation: there is not a certified manager for this facility. Obtain certification within the next 6 months.
- Light bulbs, protective shielding
Observation:Lights are not properly shielded in kitchen.
- Food employees outer clothing is clean
Observation:Upon arrival cook staff clothing is soiled. Observed staff using clothing to wipe hands on. Address proper and allowed clothing when in a food facility.
- Eating, drinking, or using tobacco
Observation:Open beverage cups above food prep areas.
- Separating raw animal foods from raw ready to eat food and separating raw animal food from cooked ready to eat foods
Observation: Cross contamination noted in prep cooler. Onions for condiments stored within raw bacon container.
- Bare hand contact with ready to eat foods
Observation:A long term plan of correction is needed. This is a repeat violation from 12/2013
- Duties of PIC
Observation: PICs are not ensuring that staff are following food safety requirements. Both cook staff touched ready to eat foods with bare hands, when discussing this with them one individual said that he would not use them because they do not fit. Cook staff clothing is soiled, cross contamination issues. Also failure to notify health department of fire-see notes below.
- Responsibility of the PIC to notify the regulatory authority.
Observation: PIC must be aware of when to report to the Regulatory Authority when illnesses occur.
- Multiuse food contact surfaces are cleanable
Observation:Stainless steel table that was damaged in fire may not be used until refurbished or the item must be replaced. Additionally, all final clean up in kitchen will need to occur after hours tonight. soot noted on ceiling in kitchen.
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation:Some of the coolers are missing thermometers.
- In-use utensils, between-use storage
Observation:Utensils are stuck between pieces of equipment. between equipment is dirty and is not being cleaned. Identify a different way to store utensils ASAP.
- Established procedures for responding to vomiting and diarrheal events
Observation: A vomit and diarrhea clean up kit is not available.
