  • Food storage - preventing contamination from the premises
    Observation: In the walk in freezer a plastic container holding ice cream and frozen pizza was stored on the floor. ALL food items shall be at least 6" off the floor to prevent contamination. CORRECTED ON SITE.
  • Plumbing system maintained in good repair
    Observation: The service sink faucet leaks. The manager called for service during this inspection. At this time the service sink is being repaired.
  • Indoor and outdoor surfaces
    Observation: Several areas need to have tile installed, such as in the front area of the cooler/freezer, and the warewashing area. Floors shall be smooth, cleanable and nonabsorbent. REPAIR AS NEEDED. --Observation: The unisex restroom has PVC laying on the floor for the water softener which drains to a floor drain. Utility service lines and pipes may not be unnecessarily exposed.Exposed utility service lines and pipes shall be installed so they do not obstruct or prevent cleaning of the floors, walls, or ceilings. Exposed horizontal utility service lines and pipes may not be installed on the floor. REPAIR AS NEEDED.

    Observation: The handwashing sink had a dirty spatula laying in the sink. A HANDWASHING SINK shall be maintained so that it is accessible at all times for EMPLOYEE use. A HANDWASHING SINK may not be used for purposes other than handwashing. Employees educated and verbalized understanding. CORRECTED ON SITE.

    Observation: Various types of frosting and glaze not labeled. SHALL label all food items taken from original packaging. CORRECTED ON SITE.
  • Air drying of equipment and utensils
    Observation: Observed an employee drying equipment that had been washed,rinsed,and sanitized.After cleaning and SANITIZING, EQUIPMENT and UTENSILS:Shall be air-dried or used after adequate draining. EMPLOYEES WERE EDUCATED AND VERBALIZED UNDERSTANDING.CORRECTED ON SITE.
  • In-use utensils, between-use storage
    Observation: A spatula and "butter brush" was stored on pizza holders which are paper. Utensils not in use shall be stored on a cleanable, nonabsorbent surface. Educated provided and employees verbalized understanding. CORRECTED ON SITE.

    Observation: The license RS/FE both expire today. The new license is not in the facility. THIS INSPECTOR SPOKE WITH LINN COUNTY PUBLIC HEALTH AND THE PAPERWORK FOR THE RENEWAL WAS SENT MAY 1ST TO THIS FACILITY. SHALL PAY FOR THE RS/FE LICENSE TODAY OR WILL BE OPERATING WITHOUT A LICENSE.SHALL COME TO LINN COUNTY PUBLIC HEALTH AND PAY FOR BOTH LICENSES TODAY. THE MANAGER STATED SHE WAS GOING TO LINN COUNTY TO PAY FOR BOTH LICENSES AT THIS TIME. WHEN THE RS/FE IS PAID FOR SHALL BE POSTED IN A CONSPICUOUS PLACE FOR CONSUMERS TO VIEW. SHALL SEND LETTER OF CORRECTION ASAP AFTER PAYING FOR THE LICENSE.
  • Storage of clean equipment and utensils
    Observation: In the outside storage area plastic and insulated cups were stored on the floor. SHALL ALWAYS store single service items at least 6" off the floor to prevent contamination. CORRECTED ON SITE.
  • Sanitizers
    Observation: The Quat sanitizer was in excess of over 200 ppm. Shall always maintain Quat sanitizer at 200 ppm.CORRECTED ON SITE.

    Handwashing signs not posted in both kitchen and restroom. Every hand sink to be used by employees must have sign posted reminding employees to wash hands.

    The following items were not date marked in kitchen line cooler: 1) Cut tomatoes 2) Home made sauces 3) Pre-cooked foods. Ready to eat food, home made sauces, and pre-cooked food if not used within 24hrs after opened or preparation must be date marked. Have 7 days from date marked to use or discard.
  • Separating raw animal foods from raw ready to eat food and separating raw animal food from cooked ready to eat foods
    Raw eggs and meats were stored above milk bottles in walk-in cooler. Raw animal food must be separated or stored below from ready to eat food.
  • Food storage - preventing contamination from the premises
    One shelving unit in walk-in cooler has lowest shelf set up less than 6" off floor. Adjust the lowest shelf at least 6" off floor.
  • Material characteristics of non-food contact surfaces
    Clean towels were stored in Mozz-prove-cheddar cheese cardboard box. Cardboard box is not reusable material. Must provide smooth and cleanable container for this purpose, such as plastic box.
  • Light bulbs, protective shielding
    The facility has other dry storage room is behind main building. Lights in the new dry storage do not appear to be shatter resistant style. Must replace light bulbs to shatter resistant provided or shielded in area where there is exposed food and food service items. REPEAT VIOLATION

Awful service by money hungry, greedy people. No hello when you walk in the door. No thank you for your business. Stone cold ugliness. The prices are so outrageous, beverages are warm, food is over cooked and over priced. Will never shop here again!
