Observation: Observed two buckets of soup, potatoes for hash browns, cheeses and raw hamburger all setting out on countertop for easy reach and ranged from 50-60 degrees. Raw hamburger and cheeses were refrigerated, soups and potatoes were disposed of.

Observation: Menu's lack proper consumer advisory language, in addition to identifying each animal food that can be cooked to order with an asterisk. Guidance document left for reference.

Observation: Observed cross contamination between handling raw hamburger, soups, and sanitizer solution. Soups were disposed of.

Observation: Thin probe tip sensitive digital thermometer is needed for use on thinly cooked foods.

Observation: Observed insects in Tangueray, Bacardi, J & B Scotch, Lauder's, Jose Quervo, Sweet Vermouth, Southern Comfort, and Jim Beam. Alcohol was set aside for disposal and shall not be served to the public.
- Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
Observation: Observed no date marking. Repeat violation.
