Observation: The person washing dishes was only washing dishes and then placing the on the drain board to dry. A rinse and sanitizing cycle shall be included in the wash process.

Observation: The back room walls were damaged and uncleanable.

Observation: The PIC did not know how cold foods in the walk in cooler shall maintain at or below.

Observation: The cooler unit in front of the meat cutting room did not have a thermometer.

Observation: The PIC did not know if the facility had a diarrhea vomit clean up procedure.

Observation: The chicken, seafood, and spring salad were dated with a month date mark.

Observation: The bleach sanitizer was 200 ppm. It was remixed to 50-100 ppm.

Observation: Several boxes were laying on the floor in the walk in freezer.

Observation: Packaged foods (seafood salad, spring salad, chicken salad, chocolate covered pretzels) did not have the ingredient lists and address of the place where it was practiced.
