8-103.12(b) Maintain critical control point monitoring records for HACCP plan as required.//Maintain up-to-date HACCP Log for sushi rice.// Employee did not log pH reading for sushi rice today (09/07/11).//COS- AFC employee had to remove sushi roll package from display case and then disassemble sushi rolls. EHS watched AFC employee properly prepare sushi rice, calibrate pH meter, and attain pH reading of rice. The pH reading was 3.96.

No sushi rice was prepared at time of inspection. Also, AFC employee stated that he had taking pH reading today but pH reading for today was not logged on HACCP form. AFC employee then had to remove sushi roll package from display case and then disassemble sushi rolls. EHS watched AFC employee properly prepare sushi rice, calibrate pH meter, and attain pH reading of rice. The pH reading was 3.96.

4-501.19 Maintain wash solution temperature at a minimum of 110 F for manual ware washing.

3-501.16(A)(2) Maintain PHF at 41 F or below. PHF in cold holding unit > 41 F due to faulty equipment. Cease use of equipment until repaired and verified by JCDH.//Both the display cooler (55 F) and sushi bar RI cooler (53 F) held ambient temperatures above 41 F. PHF (eel, crab meat, shrimp, cream cheese, and salmon) were 53 F - 55 F in sushi RI cooler. Sushi trays prepared 03/13/12 stored in display cooler were placed in another display cooler near the deli. Cease using both coolers (display and sushi RI) until repaired and verified by JCDH.//COS - PHF were discarded.
