JCDH 2.1 Register all Certified Food Safety Managers with JCDH and maintain the appropriate card and certificate in a readily accessible location. JCDH 2.2 Provide readily available documentation of food safety training for all employees at all hours of operation.//Several employees certificates were expired and current certificates were signed by a manager that is no longer employed at FSE.//FSE must update safety training binder.
3-501.16(A)(2) Maintain PHF at 41 F or below. PHF in cold holding unit > 41 F due to faulty equipment. Cease use of equipment until repaired and verified by JCDH.//Several deli meat portions, cheese slices, and tomatoes were stored in RI cooler under sandwich make line at temperatures of 59 F - 61F. These PHF items were prepped and stored on 11/02/11. Ambient temperature in RI cooler was 65 F. //COS- PHF items were discarded.- Technician was working on RI cooler at time of inspection.
