Accumulation of black residue inside of ice machine. 3-601.11 Cease the sale of food not labeled for individual retail sale as specified in law. Facility cutting cartons of eggs in half, sleeves of saltine crackers, and oreo cookies. 5-205.15 Repair plumbing. Water tank leaking. 5-205.11 Provide access to handsinks at all times. Accumulation of cans, disinfectants, cleaning supplies, and other items inside of handwashing sink. 5-205.11 Cease using handsinks for any purpose other than handwashing. 6-301.11 Provide soap at all hand sinks. The only handwashing sink that had soap available was the restroom sink. 6-301.12 Provide paper towels or drying device at all hand sinks. Only handwashing sink that had paper towels available was the sink across from the ice machine. 6-301.20 Provide waste receptacles at all handsinks with paper towels. 6-501.112 Remove dead or trapped birds, insects, rodents and other pests from control devices and premises at a frequency that prevents accumulation, decomposition, or the attraction of pests. Dead insect bodies throughout the faciltiy.

The following shelf stable products were found to be past the manufacturers date limitation: Bumble Bee chunk light tuna with a touch of lemon expired 03/16/12, Campbells cream of chicken condensed soup expired 11/05/11, and Hormel Compleats cheese and spinach ravioli expired 02/08/12. Items were removed from shelving to return to vendor. 5-501.113 Keep dumpster closed.
