4-601.11 Clean interior of ice machine as often as necessary to prevent accumulation of soil and mold. Ice dispenser has slime build up.
4-501.114, 4-703.11 Sanitize utensils with chlorine for 10 seconds or more at the following corresponding temperatures and concentrations: 25 ppm at 120 F or higher; 50 ppm at 75 F or higher; or, 100 ppm at 55 F or higher. Dish machine not sanitizing.
3-304.11 Prevent the contact of food with non-food contact surfaces, or, with food contact surfaces that are not properly cleaned and sanitized. Dish washing employee did not sanitizer equipment and utensils after hand washing. Equipment was properly sanitized at the time of inspection.
3-501.15 Cease wrapping or covering food until cooled. Cooked chicken quarters cooling wrapped at 50F. Allow chicken to completely cool in walk in cooler to 41F or below before wrapping.

Follow up at 3:45 pm on 11/30/11- returned to facility to abate nov for dish machine. Dish machine was operating properly.

3-304.14 Store wet wiping cloths for sanitizing in 50 to 100 ppm chlorine. Wiping cloth solution tested negative at main food prep area. Test sanitizer solution before using.
6-501.14 Clean restroom exhaust vent cover. Toilet room vent cover has dusty build up.
6-501.12 Clean facilities (floors, walls, ceilings, fixtures, etc.) as often as necessary to maintain cleanliness. Clean floor under reach in cooler at front counter.
