  • KDA_3_10111 - --In the WIC there was a metal sheet pan of 11 raw salmon fillets thawing in vacuumed sealed packages. The person in charge said they have been in the sealed packages for less than 24 hours. COS-slit open packages. In the dry food storage room there was one rotting yellow onion. COS-discarded.
  • KDA_3_50116A2 - --In make table 3 there was a container of sliced cheese at 53.1F. The lid to the make table was opened for 30 minutes and tempted around 3:00pm. The ambient air temperature of the unit was reading 36.0F. COS-closed the lid when it was not in use. In make table 4 (in between the grills) there was a plastic food grade container of sliced provolone cheese at 49.1F. It was above the fill line. COS-removed the excess cheese and placed it down in the container. In the RIC (below MT 5) there was corn on the cob at 55.9F. It was out in the microwave for less than an hour. It was placed back into the reach in cooler. The ambient air temperature of the unit was reading 38.6F. In the RIC (below MT 7) there was a container of corn dogs at 45.7F. They were placed in the unit around 2:00pm and tempted around 3:30pm (less than 4 hours). The ambient air temperature of the unit was reading 39.6F. COS-moved to another reach in cooler. In the Hamilton & Beach RIC, there was an open half gallon of chocolate milk at 44.4F and an open gallon of 2% milk at 47.1F. They have been in the unit longer than 4 hours. COS-discarded the milk and the person in charge adjusted the temperature gauge and now the ambient air temperature of the unit is reading 40.8F.
  • KDA_4_20211A2 - --On the shelf below a prep table there was one (1/3) plastic food grade container with a crack at the bottom (not easy to clean). COS-discarded. On a utensil rack there were 3 medium lids and 4 large lids with broken edges. There was also one (1/2) plastic food grade container with a cracked edge (not easy to clean). COS-discarded.
  • KDA_4_50111B - --The seal around the WIC door has 1.5" gap. Temperatures in the WIC are below 41F.
  • KDA_4_60111A - --On a prep table there was a white plastic tub with 6 tan plastic lids with dried food debris on them and stored clean. On a shelf below the prep table there were 5 medium tan plastic food grade containers with stickers on the outside and stored clean. There were two (1/9) and one (1/6) food grade containers with dried food debris on the inside and stored clean. There were four (1/3) plastic food grade containers with stickers on the outside and stored clean. COS-moved to the ware wash area. On a shelf below the long prep table there was a square metal produce slicer used today with food debris on the blades and stored clean. There was another produce slicer used today with food debris on the blades and stored clean. COS-moved to the ware wash area. In the ware wash area on a utensil rack there was one plastic food grade spacer with a sticker left on it and stored clean. COS-moved to the ware wash area. On the food service line there was one cast iron skillet with food debris left on the inside and stored clean. COS-moved to the ware wash area. On a utensil rack there was one (22 qt) container with a sticker left on the outside and stored clean (can hold bacteria). COS-moved to the ware wash area.
  • KDA_4_90311B - --On the shelf below a prep table there were three (1/6) and three (1/3) plastic food grade containers stacked wet/clean and not able to properly air dry. COS-moved to the ware wash area.
  • KDA_5_20515B - --The handsink across from the fryers faucet leaks towards the base when water is supplied.
  • KDA_6_50111 - --In the dry food storage room, part of the tile baseboard is missing. Around the door frame, there are holes and the frame is deteriorating. (Note: the general manager is replacing the grout between the tiled floors).
