- KDA_3_30211A1a - --In the RIC (below MT 1) on a metal sheet pan, there was 3 ready to eat bananas (2 whole and 1 cut) being stored directly next to an open package of raw bacon. COS-rearranged items. (Note: no leakage detected).
- KDA_3_50116A2 - --In RIC (below MT2) there was a plastic food grade container of diced tomatoes at 45.5F, a plastic food grade container of chorizo at 43.7F and a plastic food grade container of diced ham at 46.8F. The items have been in the unit less than 4 hours. COS-moved to the WIC. The ambient air temperature of the unit is reading 35.4F. The right door does not shut properly, the PIC called for the unit to be serviced and will get the hinge looked at. On the metal table in between the steam tables, there was a can of whipped cream (keep refrigerated) at 64.1F. It was sitting out less than 4 hours. COS-placed it back in the WIC.
- KDA_4_20211A2 - --On a shelf below the microwaves, there was a large clear plastic lid that was cracked and being used on a container to store biscuits. COS-placed clear plastic wrap over container. On the shelf above the microwaves there was one (8qt) plastic food grade container that had a crack on the side. There was also one (4qt) plastic food grade container with a broken edge. On the utensil rack (next to the hot water tank) there were 5 square and 6 rectangle plastic food grade lids with cracked edges. COS-discarded all lids and containers.
- KDA_4_50111B - --The right door to the RIC (below MT 2) does not shut properly due to the hinge. (Note: the PIC called for the unit to be serviced).
- KDA_4_60111A - --In a plastic container there were 2 knives with dried food debris on the blade and stored clean. On the metal shelf above the microwaves there was one tan square food grade container, one tan plastic rectangle food grade container, one metal rectangle food grade container, two (4qt) plastic food grade containers with dried food debris and stored clean. COS-moved to the ware wash area. On the metal shelf above the microwaves there were 2 metal food grade containers and two (4 qt) plastic food grade containers with stickers left on them and stored clean (can hold bacteria). On the utensil rack (next to the hot water tank) there were 3 white round plastic food grade lids with stickers left on them and stored clean. COS-the PIC removed the stickers and moved them to the ware wash area.
- KDA_4_70211 - --The mechanical dish machine was in use, utilizes chlorine for sanitization and it measured at 0ppm. COS-the PIC primed the machine and now it is reading 100ppm, rewashed the dishes.
- KDA_4_90311B - --On the metal shelf above the microwaves there were 4 plastic food grade containers and 2 metal food grade containers stacked wet/clean and not able to properly air dry.
- KDA_6_30114 - --There is no handwashing sign at the waitress handsink or in the men's restroom. COS-left handwashing stickers.
- KDA_7_10211 - --In the kitchen there was a round white plastic bucket with pink liquid and was not labeled. The PIC said it was sanitizer. COS-labeled bucket sanitizer.
