- KDA_3_50115A - --A container of cut lettuce with a temperature of 44 was stored in West make table. 8 containers of shredded cheese with a temperature of 51 were stored in West make table. Cook said all foods were prepared 3.5 hours ago and placed in make table. (corrected on site-all foods moved to walk in cooler for quick cooling) It is recommended to not overfill make table with foods and to immediately place containers in coolers after preparation.
- KDA_3_50118A1 - --A container of baby back ribs in walk in cooler was dated 12/1. At time of inspection, the ribs were 9 days old. (corrected on site-ribs disposed of in trash)
- KDA_4_50111A - --Handles and/or lids on both make tables and South hot box are breaking and missing.
- KDA_4_50111B - --Insulation on lid of West make table is breaking and detaching from door.
- KDA_4_50112 - --The cutting boards at kitchen make tables have deep cuts and scratches in them.
- KDA_4_60111A - --6 plastic containers stored on clean dish rack next to dishwasher had old date stickers on them. (corrected on site-containers moved to dishwasher to be properly cleaned) Can opener stored on prep table next to walk in cooler had visible dried food debris. Cook said it had been not been used for over 4 hours. (corrected on site-can opener moved to dishwasher for proper cleaning)

- KDA_5_20511B - --A plastic container of water was sitting in basin of handwashing sink next to make tables. (corrected on site-container removed)
- KDA_5_501115 - --The area around outside trash receptacle has accumulation of trash and soil.
- KDA_6_20211A - --Light bulb above walk in cooler entry door does not have shield.
- KDA_6_30111 - --Handwashing sink next to make tables do not have supply of soap. Cook said it was broken and new dispenser should arrive in a few days. (corrected on site-handwashing sink located in back of kitchen is currently being used as an alternate)
- KDA_6_30114 - --All three handwashing sinks in kitchen area do not have handwashing signs.

- KDA_7_10211 - --Two unlabeled spray bottles with clear and blue liquids were stored on chemical rack next to mop sink. Manager said one was all-purpose cleaner and the other was window cleaner. (corrected on site-bottles properly labeled) This was the only chemical violation in facility during inspection visit.
