  • C/*4.5/4.6 Observed a soiled pot with a pooling liquid substance on the stove-top. Food contact surfaces shall be washed, rinsed, and sanitized, and shall be clean to sight and touch. PIC relocated pan to the three-vat sink.
  • R/6.4 Repair the damaged floor tiles throughout the bar area, damaged wall surface in the women's restroom, and damaged floor tile surrounding the floor drain under the hand sink, hole in the wall above the base cove by the doorway to the storage area with the coolers, to provide cleanable surfaces.
  • R/6.4 Clean the ceiling surface in the side room above the refrigeration and freezer units which is soiled and stained.
  • R/6.2 Provide a hand washing sign in the men's and women's restroom.

  • 3.2 Observed soiled linens on the reach-in unit from the previous day. Place soiled linens in the appropriate bin to be laundered.
  • 6.2 Repair the non-functioning light fixture by the three vat sink, to provide 20 foot-candles of lighting.

  • - pooling water/substance on top of the grease trap,
  • - hood filters above the fryer,
  • - larger amount of ice accumulation on the back of the Arctic Air refrigeration unit,
  • - the floor and floor/wall junctures in the bar area,
  • - bottom of the Arctic Air Freezer unit
  • - the wall surface under the hood is soiled,
  • - interior of the bottled beer cooler across from the register, which are soiled or contain grease or ice accumulations.
  • 4.1/9.0 Observed a non-commercial toaster and Breville panini grill on premise. Remove non-commercial equipment from premise.
  • 5.1 Repair the leaky faucet on the kitchen hand sink. Plumbing shall be maintained in a state of good repair.
  • 6.1 The lacking light shield in the kitchen area, to prevent contamination of foods.
  • Note: Extra fryer on-site is used as a back-up during high demand times.

  • *5.1/2.4/6.2 Observed the kitchen hand sink contained a bleach bottle and a string of lights and was lacking paper towels. Also the bar hand sink was blocked the a garbage can and was lacking soap. Keep the hand sinks accessible and provide an adequate supply of soap and paper towels to promote proper hand washing.
  • R/6.4 Repair the damaged floor tiles throughout the bar area, also in the men's restroom and women's restroom, to provide a cleanable surface.
  • R/4.5/6.4 Clean the floor and floor/wall junctures in the bar area which are soiled.
  • R/6.4 Clean the ceiling surface in the side room above the refrigeration and freezer units, and under the ice machine and mop sink, which are soiled and stained.
  • R/4.2 Provide a QAC test kit the assure 200 ppm is maintained, current test kit is damaged

  • R/*7.1 Store chaffing fuel separate from the spices, to prevent contamination.
  • C/*3.0 Observed the corned beef was discolored and the white American cheese contained a mold-like growth. Food shall be safe wholesome and adulterated. These food items were discarded today.
  • *3.0/3.4 Observed raw chicken wings in pooling water at 62.6°F in the middle-vat of the three vat sink from the previous day.
  • Foods shall be thawed under running draining water, and no portion of the food shall be held above 41°F. Discard this food item to prevent food borne illness.
  • *2.4/6.4 Relocate ibuprofen in a separate area from food products to prevent contamination, or accidental misuse.
  • R/6.4 Repair the damaged wall surface behind the preparation sink, the reach-in unit, three-vat, hole in the ceiling by the light fixture, and damaged floor tiles throughout the bar area, ceiling above the food storage rack, damaged floor tiles in the men's restroom, damaged wall surface in the women's restroom, to provide a cleanable surface.
