  • *3.4 - Several items in deli case were not date marked at all or were beyond 7 day time frame. Specifically, muenster cheese, dated 3/15 and turkey breast with no date marking at all. A bag of hot dogs and a bag of cooked chicken were in the walk-in cooler without date marks on them. Foods not used within 24 hours shall be dated the day of preparation and shall be used or discarded in seven days. The Muenster cheese and the hot dogs were discarded. Reportedly, the bag of cooked chicken was prepared the previous day and was going to be used to make wraps today.
  • *3.4 - Creamer in Coffee Mate dispenser had a temperature of 50°F. Potentially hazardous foods shall be maintained at 41°F or below to prevent possible growth of harmful bacteria which may cause a foodborne illness.
  • *5.1 Food preparation sink needs an air gap going to the floor drain. Remove last turn on pipe going into floor drain in order to provide this air gap. Pipe cannot sit down inside this floor drain.
  • *901:3-4 - The following jars of baby food were offered for retail sale with dates beyond the manufacturer expiration date:
  • 6 jars of Gerber bananas, dated 1/19/12, and 1 jar dated 2/24/12
  • Gerber applesauce, 8 jars dated 12/11/11, 8 dated 2/10/12, 3 dated 1/15/12, 1 dated 1/14/12, and 2 dated 1/20/12.
  • 2 jars Gerber sweet potatos, dated 2/1/12
  • 2 jars Gerber green beans, dated 1/5/12
  • 1 jar Gerber peas, dated 3/8/12
  • 3 jars Gerber carrots, dated 1/20/12 and 1 dated 2/12/12.
  • Infant formula, baby food, etc. may not be sold beyond the manufacturer expiration date. Manager removed baby food from shelves.
  • 4.5 The racks holding beverages in the reach-in coolers are dirty, especially under Pepsi, Coke and tea products. Equipment shall be free of an accumulation of dirt, dust and other debris.
  • NOTE: The following food items were offered for retail sale beyond the manufacturer use by/sell by dates:
  • 6 boxes of Betty Crocker yellow cake mix, dated 10/20/2011
  • 1 box of Betty Crocker devils food cake mix, dated 12/23/11
  • 3 boxes of Pillsbury pie dough mix, dated 1/1/12 and 3 boxes dated 2/17/12.
  • 2 Smiths chocolate milk, dated 3/22/12
  • 1 box of Kellogg's Raisin Bran, dated 3/7/12
  • 3 boxes of Triscuits, dated 1/27/12
  • Check and rotate stock on a regular basis.

  • 6.4- Ceiling tiles above produce cooler are black and moldy and hanging from ceiling. Reportedly these tiles are scheduled for replaced this weekend.
  • Note: Food preparation sink has been installed. According to plumbing inspector plumbing permit is not needed but he pipe going to the floor drain needs an air gap. Remove last turn on pipe going into floor drain in order to provide this air gap. Pipe cannot sit down inside the floor drain.

  • 3.4- Deli meats in deli case were not all date marked and ham salad and chicken salad made on site were not date marked. Readyto- eat potentially hazardous foods shall be date marked when opened or prepared and consumed within seven days.
  • 3.0- Moldy and slimy lettuce and tomatoes in the walk-in cooler used for sandwiches. Foods shall be safe and unadulterated.
  • Discard moldy foods.
  • 901:3-4-15- Gerber baby wheat cereal with an expiration date of March 23, 2011 was for sale on reduced foods rack. Do not sell expired baby/infant foods, formula, or cereal. Cereal was removed from shelving.
  • *3.2- Raw sausage links stored next to lunchables in the retail case. Store raw animal foods separate from ready-to-eat to prevent possible contamination.
  • 4.4- Hot case heat lamps are not working. One side of case had an ambient air temperature of 104°F. Repair unit in order to maintain foods at 135°F and above. Operation is aware unit does not work but is still being used.
  • 5.3- Provide an indirectly drained food preparation sink to wash potatoes. Sink on site but not hooked into plumbing.
  • 3.5- Several foods did not have ingredient labels including frozen donuts, little hugs fruit barrels, and macaroni and cheese.
  • Provide ingredient labels for these foods.
  • 6.4- Fly strip over three compartment sink. Relocate fly strip away from food and equipment.
  • 6.4- Floor and wall junctures have debris build-up in deli cooler and large walk-in cooler. Clean the floor.
  • 6.4- Ceiling tiles above produce cooler are black and moldy. Replace damaged ceiling tiles and repair any leaks.
  • 3.5- Do not cover manufacturer dates with price tags. Baby food dates and canned food dates were covered by price tags.
  • C/*4.6- Sanitizer bottle for premix on three compartment sink was empty. Food contact surfaces shall be sanitized. Bottle was replaced during this inspection.
  • Notes: Foods throughout were past the sell by date include but not limited to: clam chowder October 2010, Thousand island April 25,
  • 2011, cake mix August 2, 2011, slim Jim July 16, 2011, wheat thins June 2011.
  • This office received a complaint regarding the above operation. Reportedly complainant has received undercooked chicken on several occasions. Discussed with food employee and chicken is reportedly cooked to 165°F but chicken may be cold because hot case is not functioning properly.
  • Breading and batter water is discarded at least every four hours.

  • R6.4- Clean the wall and floor around the three compartment sink it is dirty.
  • R6.2- Replace burned out lights in the deli and back area.
  • 6.4- Replace missing ceiling tile in the back storage room.
  • 3.5- Provide ingredient labels for packaged retail items such as macaroni and cheese, barbecue chicken, candy hearts, and popsicles.
  • C/*3.0- Lemons on retail floor were rotten/excessive mold growth. Food shall be safe for human consumption.
  • 3.2- Raw bacon stored next to sauerkraut in retail case. Provide a divider between raw animal foods and ready-to-eat foods to prevent possible cross contamination.
  • Note: Discussed complaint with manager regarding meat being sold past the sell by date. She was aware of the complaint. No foods observed at this time were past the sell by date.

  • 4.0- Garbage bags used for chicken breading. Do not use garbage bags for food storage. Garbage bags are made of recycled materials and may be treated with insecticides. Provide a food grade bag.
  • 6.2- Replace the burned out light in the deli area.
  • 6.4- Clean the wall around the potato cutter and floor under the three compartment sink.
  • R4.5- Walk-in freezer has ice accumulation. Remove ice and make any necessary repairs.

  • 8/8/10, yogurt 9/10/10, Campbell's tomato soup 7/14/10 and 10/13/09, Ritz crackers 8/29/10, different variety of crackers
  • 6/2010, Honey nut scooter cereal 7/17/2010. These foods were removed from shelving.
