- 3.4- Foods throughout were not date marked or were past the seven days consume by date. Ready-to-eat potentially hazardous foods (deli meats and sausage gravy) shall be date marked when opened and consumed within seven days.
- 2.4/6.2- No paper towels available at the restroom hand sink. Provide an adequate supply of paper towels to facilitate hand washing.
- 3.4/4.6- Raw chicken breading machine had breading in it from the previous day. Machine shall be washed, rinsed, and sanitized every four hours since it is stored at room temperature.
- 4.4- Quaternary ammonium chloride sanitizing solution in the wiping cloth bucket had a residual over 500 ppm. Maintain QAC sanitizer at 200 ppm to avoid leaving a toxic residue on food contact surfaces.
- 3.2- Raw bacon stored in the same container as hotdogs in the retail cooler. Store raw animal foods separate from ready-to-eat to prevent possible cross contamination.
- Notes: Discussed with new owner the requirement to purchase prewashed potatoes since a food preparation sink has not been installed.
- Large retail case has been removed from retail floor.
- Vanilla wafers had a sell by date of October 27, 2010. These were removed from the shelving.

- 4.4/3.4- Sandwich case had an ambient air temperature of 46°F. Chopped ham in case at 48°F, cheese 48°F, and macaroni salad
- 51°F. Repair unit in order to maintain 41°F and below. Reportedly unit might not be repaired and turned off and no longer used.
- Sandwiches would be put in a different unit and meat would not be sold by the pound.
- *7.1- Cleaner stored under front counter next to saran wrap. Relocate chemicals separate from food and single-use items to prevent possible contamination.
- 4.1- Provide an accurate thermometer in the sandwich case. Thermometer reading 60°F and actual temperature is 46°F.
- R4.5- Shelving in the walk-in cooler is dirty. Clean the shelving.
- 3.5- Pudding cups are labeled not to be sold for individual sale. Provide ingredient labels for pudding or do not separate from package.
- Notes: Removed foods from shelving past their sell by date corn 3.10, oatmeal 3/10,10/09, French dressing 12/08. Check and rotate stock regularly.
